If you were a previously established patient with me at a different location please request your medical records by calling the main hospital number (406) 543-7271 and request to be transferred to the medical records department. They can fax them directly to me at (949) 864 3694. Please also fill out the below form and i will call you to schedule an appointment once your records are received.

If you are a new patient that i have not previously seen, please fill out the above form, and we will contact you for a free consultation. If you have previously seen a psychiatric provider please request their initial evaluation, notes from the last year, a list of all medication trials, a current medication list, along with a problem list and medication list from your primary care provider( if you have chronic medical problems or take other medications) and any lab work and have them faxed to (949) 864 3694.
If a primary care provider has been providing your psychiatric care please request a current medication list, a problem list, lab work for the last year and any pertinent psychiatric notes from the last year and have them faxed to (949) 864 3694. Also fill out the below form.
